PARENT REUNIFICATION

Dealing with social services systems and the courts can be very overwhelming for families. This service offers one on one support to a parent trying to work through the process of reunifying their family. The worker can provide support, be a role model, link parents to services, and assist them in understanding and following through with court order to meet the guidelines provided in order to reunify their family.

The mentor completes a needs assessment to understand the parent and families needs in order to re-establish child-parent health relationships and custody. Support from the worker can include supporting the parent in attending supervised visitation, connecting to therapeutic services, finding employment, stable housing, among other needs which would be identified at the assessment. An individualized plan would be established in collaboration with the parent taking into account Social Services and Court requirements and/or recommendations. The ultimate goal is reunification.
